If you have Googled "wisdom teeth removal," you have almost certainly run into the horror stories about Dry Socket (Alveolar Osteitis). It is the number one complication patients ask us about, and for good reason—it can be quite painful.
The good news? It is also highly preventable.
To understand how to prevent it, you first need to understand what it is. When a tooth is removed, your body naturally forms a blood clot in the socket (the hole where the tooth was). Think of this clot like a scab on a scraped knee. It protects the exposed bone and nerves underneath and acts as the foundation for new tissue to grow.
Dry socket happens when that blood clot is dislodged or dissolves too early. This leaves the bone and nerves exposed to air, food, and fluids.

The "Big Three" to Avoid (The 3 S's)
During the first 3 to 5 days after surgery, the clot is most fragile. You need to avoid any action that creates suction or pressure in the mouth.
1. No Straws (Suction) This is the golden rule. The suction required to pull liquid up a straw creates a vacuum in your mouth that can physically pull the blood clot out of the socket.
The Fix: Drink directly from the cup or use a spoon for smoothies/milkshakes for the first week.
2. No Smoking or Vaping (Suction + Chemicals) This is a double threat. First, the inhaling action creates suction (just like a straw). Second, nicotine constricts blood vessels, which slows down blood flow and makes it harder for your body to maintain the clot.
The Fix: We know it is difficult, but try to avoid smoking or vaping for at least 72 hours (ideally 5 days). Using a nicotine patch during this time can help reduce cravings without the risk to your healing.
3. No Spitting (Pressure) When you rinse your mouth, your instinct is to forcefully spit into the sink. This force can dislodge the clot.
The Fix: Instead of spitting, just open your mouth over the sink and let the water drool out naturally. Gravity is your friend!
Diet and Hygiene: Handle with Care

For the first 24 hours, do not rinse your mouth at all. Let the area settle.
After 24 hours, you can start using warm salt water (1/2 teaspoon of salt in a cup of warm water). Do not swish vigorously. Instead, gently tilt your head side to side to wash the area, then let the water fall out.
Stick to Soft Foods Crunchy foods like chips, popcorn, nuts, or seeds are dangerous right now. Small fragments can get lodged in the socket and irritate the clot. Stick to:
Yogurt and pudding
Mashed potatoes
Scrambled eggs
Lukewarm soup (avoid boiling hot liquids on the first day)
